Job Title: Clinical Intake Specialist/TherapistSupervisor:
Managing Director/Clinical Supervisor -To help clients reach the
goal of recovery and independence by exhausting the full spectrum
of individual, group, and family therapeutic services with
fidelity. ROLE (What I do)
Responsible for Performance
Management of Key Clinic Performance Indicators related to
admissions and intake.Responsible for the referral pipeline and
management of all referralsResponsible for Client Registration
System (i.e., demographics, financials, insurance for
eligibility)Responsible for overall intake operations ensuring a
collaborative effort between Intake Care Coordinator Assist w/
Triage & Admissions/Re-Admissions coordinating initial course of
action.Assist with Front Office Operations/Customer
RelationshipsCollaborates with Practice Manager to assist with
Business Development/Community Liaison Support to help build
business and referral pipelines.Responsible for understanding both
treatment options and business operations (impact on business and
bottom line).Carry a Counseling caseload (20-28 hours of direct
care). Assist with Groups as needed. Responsible for the
Performance Quality Improvement (PQI) ProgramResponsible for
monitoring and ensuring implementation of all clinical
documentation - Documenting all interactions with staff,
individuals/youth, family, involved agencies, schools, etc.,
concisely summarizing those interactions, timely.Attends weekly
agency clinical supervision and weekly treatment team
meeting.Attends Licensure supervision as applicable. Communicates
regularly with treatment team members and family including referral
sources, Site Supervisor/Coordinator and Business Coordinator, and
owner as appropriate.Ascribe and use trauma informed approach to
treatment in individual, group, and family therapy (i.e. counsel,
consult, mediate, facilitate, advocate, advise, collaborate)
Develop, direct, and monitor treatment for all clients and
familiesEngage family in services, and work to support referral
agents' permanency plan and/or stability plan.Assist families with
parenting skills training, basic life skills, assess family
roles/relationships, communication patterns, and conflict
management. Maintains current records, up-to-date case records, and
completes clinical documentation.Be aware of, and follow, state law
and respective professional codes of ethics and state reporting
laws.Maintains professional boundaries with client, family members,
legal guardians, community resources, referral sources, and
community partners.Provide individual, family, and group behavioral
health therapy services for children, adolescents, adults, and
families in an outpatient office setting by utilizing the most
appropriate evidence-based practice(s) to address clients'
needs.Support and direct the treatment team which includes the
behavioral health professional (BHP), case manager, adult clients,
parents and/or guardians, the child (when appropriate) and other
individuals identified by the treatment team in the development and
ongoing implementation of each client's individual service plan,
goals and objectives. Develop and implement treatment plans for
clients based upon clients' areas of need, diagnoses, and
symptoms.Maintain multi-disciplinary working relationships with
clinicians and other related professionals in the community. This
will include making appropriate referrals, sharing of clinical
information within HIPAA guidelines and providing clinically
appropriate multi-disciplinary interventions and
collaborations.Complete timely and accurate documentation, prepare,
and complete clinic progress notes, assessments, reports, schedules
and other material required as a function of the position.Fully
participate in staff development opportunities offered to enhance
professional growth, especially to maintain license, certification,
or identified program performance skills.Implement and maintain the
Behavioral Healthcare program's philosophy, principles, and
procedures in all facets of performance related to the assigned
treatment program.Interviews, assesses and completes all intake
documents for new clients and their families as applicable.
Develops a Treatment Plan for each client outlining strengths,
barriers, treatment goals, objectives and a crisis intervention
plan. Provides all necessary individual and family therapy as
indicated by the goals and objectives outlined on the Service Plan
and the Individual Treatment Plan for each client/family in keeping
with KY Medicaid regulations. Participates, supports and oversees
all clinical aspects of service planning meetings as required and
in keeping with KY Medicaid regulations. Evaluates clients'
progress on treatment goals and objectives in conjunction with the
treatment team, client and family. - Modifies and updates Treatment
Plan as needed. Provides referral and linkage to other community
resources to ensure that treatment goals are met. - Acts as a
liaison between the agency and those community resources. Maintains
regular contact with each client and their family as dictated by
the Service Plan and KY regulations. - Completes documentation
including intake and assessment forms, individual and collateral
therapy notes, treatment plans, within regulation guidelines and
time frames. Submits Contact Logs and coordinates clinical notes,
time sheets and Individual Treatment Plans and assessments as
required. Provides clinical direction and support to support staff,
case managers and other staff in all decisions that affect the
client's treatment.Assists in crisis intervention efforts to
maintain the safety of the clients.Coordinates discharge planning
as an ongoing process of treatment. Provides referrals to
alternative aftercare prior to discharge of clients.Keeps Clinical
Supervisor apprised of all significant treatment issues within the
program.Maintains on-call responsibilities for crisis intervention
and clinical direction as needed.Attendance is crucial to
positionFosters an environment of collaboration, professionalism,
patient/colleague safety, quality care, continuous improvement and
reward and recognition.PROCEDURES (How I Do What I Do)
Manages assigned caseload completing
a minimum of 60% per week Coordinates specific client's treatment
with the assigned case manager, Family Navigator, and BHP Team
Coordinator DOCUMENTATION- Follows the opening paperwork,
assessments, and 30-day plan; Develops an Individual Service plan
(ISP) utilizing trauma-informed approaches; follows all agency,
contractual, and legal documentation requirements in the mandated
time frame. Completes case notes within 48 hrs. DIRECT SERVICE-
Psychotherapy- Provides both an individual and family counseling
visit weekly following their ISP; facilitate therapeutic
requirements; Maintains on-call responsibilities for crisis support
and intervention
Assessment/ReassessmentMaintenance/Transition/Planning with
assistance/approval from Clinical SupervisorClinical Reviews and
coordinated discharge planning Develop discharge summary and
complete discontinuation agreement and transition plan for assigned
caseloads in prescribed time frame for administrative approval.
Decisions on establishing goals and formal action plans will always
be made in conjunction with the client and case manager/treatment
team and reviewed with the clinical supervisor for approval.
Complete internal and external referrals for serviceADMINISTRATIVE-
Adheres to ethical standards of professionalism; Maintains an
updated daily itinerary/centralized calendar for all work and
services.TRAINING- Participates in on-going internal and external
training as required for continuous growth of clinical skills. -

Member of the BHP Team
Administratively supervised by Office Manager/Business
Coordinator.Participates in licensure supervision per agency
requirements (as applicable)Interfaces with FCCS business team.Case
Review committee Member Collaborates with families, referral
agents, and other interested partiesNetwork with external
therapists, referral sources, parents, and other community
resources.Assists with case group coverage across teams as needed.

Must be 21 years of ageMaster's
degree or above in a clinical field of study related to behavioral
health from an accredited school.Licensed independently in Kentucky
or willingness to obtain independent license in one of the below
licensures while under supervision from an organizationally
assigned clinical supervisor: PhD, PsyD, LCSW, CSW, LPCC, LPCA,
LMFT, MFT, LPAT, LPATA, LCADC, or LCADCA.Minimum 1 years'
experience working with adults, children and/or families, substance
use. (Relevant education, training, experience, knowledge, and
expertise to meet the needs of clients through individual, family,
and group therapy)Have a current valid driver's license with a good
driving record for at least the past 5 yearsMust have reliable
transportationMust have current auto insurance (required coverage

organizational skills.Overall sound critical thinking and
decision-making skills with the ability to carry out decisions and
tasks.Requires excellent written and oral communication skills.Must
be able to independently manage time and be flexible.Provide

boundaries.Therapeutic and crisis intervention skills
needed.Knowledge of mental health diagnosis (DSM V) and ability to
utilize effectively a range of therapeutic techniques.Must be able
to function in a leadership capacity while working as part of a
team.Ability to liaison with other professionals is
