Job Description:

(Physician/MD qualifications required) Family Practice - Family Practice Physician needed for outpatient clinic only position

Base Salary Range:$240,000 annualized over 26 bi-weekly pay periods
Production Bonus: Monthly potential with no cap$40 per encounter in a one month period greater than 310
CME: $2,800 per year reimbursement
License & Association Fees: $1,800 per year reimbursement
NP Collaborative Agreements: $1,000 per KBN agreement annually up to $5000 maximum.

Health-Dental-Vision-Life: Organization pays a portion of all 401K Plan: $1 for $1 match on first 3% of income and .50 cents on the $1 on the 4 and 5% for a total max match at 4% of your income. This is a Safe Harbor Plan so you are vested on the company match immediately.

Time Off:240 hours of PTO, 10 Paid Holidays (including your birthday) Earn extended illness time: up to 6 weeks
Loan Repayment: NHSC loan repayment eligibility

Call Rotation:Limited second level call via telephone, typically 1 in 6 week rotation.

Typical Schedule:Physician will be scheduled regularly for 40 hours per week in the office which may include Saturdays and evenings. Physician may need to work additional hours, as necessary to complete Physicians duties.
