This position coordinates total nursing care for patients, participates in patient and family teaching, acts as a patient advocate and works as part of a team with ancillary and non-nursing departments. - Assesses the patient's condition and nursing care needs - Sets goals and prescribes appropriate nursing interventions to meet physical, psychological, social and rehabilitative needs of patients - Develops individual nursing care plans - Implements care plans and makes revisions as needed - Evaluates patient care outcomes - Collaborates with physicians and other hospital employees - Administers medications, treatments and therapies. - Communicates with patients and oversees provision of care. - Provides patients and/or family with instruction and education relevant to condition. - Accurately documents nursing observations, assessments, medications, treatments, and other care given.
